I have a brother named Allen and another named John A. (for Alva). When I  
ordered my mother's death certificate, the name of the person giving the  
information was John Allen - no comma. I can see how the error was made, but I 
 was surprised. This is a small town so the funeral home people knew both 
of my  brother.
