If you're recently sent in an incomplete Certificate of Records
Destruction (RM-3 Form), you will have noticed that we've begun
returning forms to be corrected and resubmitted. Before you submit a
RM-3 Form, ask yourself:
* Are you using the most current form? Use the form available
online at www.lva.virginia.gov/agencies/records/forms.asp.
* Have you identified a valid records series number? If you are
using a General Schedule, make sure you're referencing the most recent
version, available at
www.lva.virginia.gov/agencies/records/retention.asp.
* Is the form filled out completely, including date range, volume,
and method of destruction?
* Has the form been signed by an approving official, Records
Officer, and someone who witnessed destruction of the records?
