A series was inadvertently omitted during the recent overhaul of GS-21, Public Schools.  The focus group reconvened to address the series and proposes the following changes:

 

Series 008195, Clinic Records: Non-Treatment Related will be made defunct in favor of the two new series below which separate sign-in sheets from emergency/health information sheets (they are both currently included under 008195 but now require different retention lengths and cut-off events:

 

XXXXXX, Clinic Records: Sign-In Sheets

1 year after end of academic year

This series documents clinic visits by students.  This series may include, but is not limited to: sign-in sheets containing no medical information.

XXXXXX, Clinic Records: Emergency and Health Information Sheets

0 years after superseded, obsolete, or rescinded

This series documents student emergency and health information and may contain parent and doctor contact information and lists of medical conditions and medications prescribed.  This series may include, but is not limited to: emergency and health information sheets.
