Well, let's clarify Amelia's response just a bit.  She is correct that submitting a completed RM-25 to the LVA is required, but only when designating a Records Officer.  One is not required when designating the Approving Official...that is a matter of internal agency/locality/work unit policy and is not kept on record by the LVA.

The approving official would be whomever your records management policy designates, and that usually depends on the size of the agency.  Often it is the agency head, or someone designated by the agency head.  If the agency/locality is rather large and the records functions are more decentralized, then the head of each unit may well be the approving official.  But, again, the answer should be found (or placed) in your policy.
