Start by calling him/her and asking permission.  Assure the author that you intend NO other usage of his work, but that you do need that pic in order that your work be complete.

Approach that writer gently, since there is no requirement that he/she permit you to make ANY usage of his work.

I would first search Google just in case he got the pic from still some one else.

  This would be a good question for a published author like Paul Drake to
  answer. In a book that I own there is a picture of one of my ancestors. I
  would like to copy this picture for use in my family history project. What
  is the correct way to secure permission to copy this picture or to quote
  parts of the text? The book has a 2002 copy write statement.
