I would think it means exactly what it says -- he settled on the land in 1776 but he didn't hold official title to it until this grant in 1784.

  Description: 140 acres on the River Ohio and Wheeling Creek including his settlement made in 1776.

  What does "including his settlement made in 1776" mean?
