I suspect what you are seeing is an example of a 'Tee name', a Scottish
naming practice to distinguish between two persons of the same name who
live in the same locale. They can even be passed down from father to
son. I have only recently tumbled to this tradition, but it explains
the amazing variety of Perry (my research interest name) first names in
some counties of NENC. In one county, there were so many Jerry
(Jeremiah) Perrys, that they appear in deeds and other documents as
Ridge Jerry Perry (presumably lived on a ridge), and Speckly Jerry
Perry (freckles?). Flat Perry is a name in my line for an ancestor
actually named John. Here is a website that explains the practice.

Subject: [VA-SOUTHSIDE] Tax Lists notations
We get several emails a month concerning the meanings for the various
notations for names in the tax lists. Most of them describe an
occupation, a location, etc. While working on the Wythe County Personal
Property tax lists we ran across one notation that caused a chuckle or
two. It is for a Jacob King. Here's the 2 listings:
King, Jacob
King, Jacob (Hairy)
I guess even the tax collector could have a sense of humor !!
