Good afternoon,

 

I need some help to determine what schedule an Assumption of Risk Form would
apply to.  Basically it is a form that we have participants sign when they
are participating in the "internship/externship/clinical program" at an
external facility but associated with our College.  Any help that anyone can
give would be greatly appreciated.
