Want to make submitting destruction forms to LVA easier? Use the Word 
templates online to enter basic information and then save the form to your 
hard drive. The next time you need to submit a destruction form, the basic 
information will be there for you!

The Certificate of Records Destruction (RM-3 Form), and all other forms, are 
available at: http://www.lva.virginia.gov/agencies/records/forms.asp

Please submit forms that are completed in full and type-written (except 
signatures).
