Allen County will copy for a fee and while I don't remember how much, it's
not large.

Seaarching OCLC I find two institutions listed as owning _photocopies_ of
this pamphlet--Arizona State Library, Archives and Public Records anAllen
County Public Library, Indiana.

If requesting a copy through your public library's Inter-Library Loan, this
OCLC record number may be of help to the processing librarian:  OCLC13944750

I do not have access to the ILL policies for either of these institutions,
but at only 5 leaves, I _suspect_ one of them may be able to photocopy for a
fee (archival material generally requires special care and handling to
reproduce so it's not an ordinary photocopy job...)
